No, family members are only insured when travelling with the cardholder.
No, family members do not need to be listed on the policy. If they are the victim of an insured medical emergency, they will only need to present documents confirming their relationship to the cardholder.

In order to write the claim, it is necessary to present: the insurance policy (reference/QR code); invoice and payment receipt for medical services rendered; a medical certificate and doctor’s report indicating the patient’s name; diagnosis and list of services rendered; prescription with doctor’s stamp; a payment receipt with the price of each medicine.
